As the only community health centre participating in this Hypertensive Management Initiative, Sandwich CHC's primary health team is collaborating with local pharmacies to better support patients and improve their health. In addition to their regular visits to the Sandwich CHC, participants of this study will receive educational materials and tools so that they can better self-manage their blood pressure.

 

The Sandwich CHC is currently seeking people interested in participating in this patient program and study. Persons rostered with the Sandwich CHC, who are over 18 years of age, and who have a diagnosis of high blood pressure or have had an elevated blood pressure reading in the past are potentially eligible to participate.
