The Sandwich Community Health Centre's primary care team is a collaborative team of professionals comprised of physicians, nurse practitioners, nurses, social workers, registered dietitian, chiropodist, massage therapist, pharmacist, senior's programmer, and support staff. The Centre serves West Windsor, Ontario and has a catchment area. The area is bordered by the Detroit River, Crawford Avenue, Tecumseh Road West, Huron Church Road, Malden Road and Armanda/Broadway Roads. Click here to view the catchment area map.

Be involved in your healthcare. Speak up if you have questions or concerns about your care.
Tell a member of your healthcare team about your past illnesses and your current health condition.
Bring all of your medicines with you when you go to the hospital or to a medical appointment.
Tell a member of your healthcare team if you have ever had an allergic or bad reaction to any medicine or food.

Speak up if you have questions or concerns and if you don't understand, ask again. It's your body and you have a right to know.
Pay attention to the care you are receiving. Make sure you're getting the right treatments and medications by the right healthcare professionals. Don't assume anything.
Educate yourself about your diagnosis, the medical tests you are undergoing, and your treatment.
Ask a trusted family member or friend to be your advocate.
Know what medications you take and why you take them. Medication errors are the most common healthcare mistakes.
