Client Enrolment
Description:
Sandwich Community Health Centre has joined in a partnership with the Ontario Ministry of Health and Long-Term Care in support of assigning clients to a Primary Care Provider to improve the way health care is delivered in Ontario.
Your Responsibilities:
Assist clients in completing the Client Enrolment Form and Consent to Release Personal Health Information.
Responsible for collecting data related to the number of new clients enrolled.
Responsible for sending a copy of the Client Enrolment Form to the Ministry of Health and Long-Term Care.

Filing clinic copy of Client Enrolment Forms.
Learning Opportunities:
Increase knowledge about Community Health Centres and the services they provide.
Obtain improved understanding of the Ontario Ministry of Health and Long-Term Care enrolment guides.
Improve communication skills/techniques with clients.
