Client Bill of Rights / Responsibilities

At the Sandwich Community Health Centre, we believe that it is important for clients to know, understand and exercise their rights and responsibilities.

 

 

CLIENT RIGHTS

Sandwich Community Health Centre clients have the right to:

  • Be treated with consideration, courtesy and respect in a manner that fully recognizes your dignity, individuality and cultural background.
  • Privacy for your personal needs.
  • Information concerning your medical condition in terms you can understand.   You have the right to review your chart in the presence of a SCHC staff member in a timely schedule.
  • Participate in decisions regarding your care.
  • An explanation about your treatment and tests including benefits, risks and alternatives.
  • Agree to or refuse ‘consent for treatment’, including medication, and to be informed of the consequences of agreeing to or refusing treatment.
  • Confidentiality of all information.  Have all personal information remain confidential unless required by law to be disclosed.
  • Know the identity and profession of those responsible for your care.
  • Express any concerns without fear of negative results.
  • Persons with special needs or handicaps have the right to reasonable accommodation in accordance with the law (Ontario Human Rights).

CLIENT RESPONSIBILITIES

As a Sandwich Community Health Centre client you have the responsibility to:

  • Participate with all caregivers in your treatment and rehabilitation.
  • Provide accurate information regarding your medical condition.
  • Accept responsibility for the consequences of refusing treatment, cancelling or not showing up for appointments.
  • Be considerate to all those providing care and to other clients.
  • Be responsible for all personal property and valuables.
  • Observe the Centre’s rules and regulations.
  • Participate in planning for follow-up care outside the Centre.
  • Voice any concerns first to the health care provider involved or the manager if necessary.
  • Accept that the Centre cannot assume the responsibility for all uninsured financial obligations.
