Why See a Social Worker?
Social workers at Sandwich Community Health Centre can help with a wide variety of concerns such as anxiety, depression, abuse, anger, relationship troubles, grief, stress, illness, self-esteem, sexuality, substance abuse, financial difficulties, employment difficulties, applications, and many other areas that may be of concern to our clients.
Services We Offer
Counselling: Assessment, support, and help with emotional, social and practical problems. We work with individuals, couples, families, adults, and children.
Advocacy: We can try to help you obtain the services you need. We can speak on your behalf, at your request.
Case Conferences: We can meet with you and other professionals to plan how to work together more effectively.

Referral and Information: We can help you find out what you need to know and where to go for help.
How to Access Social Work Services
Social workers at Sandwich Community Health Centre accept internal, external, and self referrals for counselling services. This means other staff at Sandwich Community Health Centre, other service providers, as well as yourself can refer you to social work. Once a request has been made for social work services, a social worker will call you within one week of receiving the referral. Emergencies or urgent cases will take priority and will be contacted sooner. A social worker will ask for more information about your counselling needs and then set up an appointment with you.
Walk-In or Emergency
In case of an emergency or crisis we will do our best to see you immediately. However, we cannot guarantee that a social worker will always be available. If you find yourself in a crisis situation and a social worker at Sandwich Community Health Centre is not available, you can call the 24 hour crisis line through the Community Crisis Centre at Hotel Dieu Grace Hospital - 519-973-4435.

If necessary, social workers at Sandwich Community Health Centre can provide help over the phone. However, it is best to come in for an appointment.
Your Rights
When you see a social worker you have the right:
- To confidentiality.
- To be treated with respect.
- To determine what kind of help you want and who you will get it from.
- To review anything written about you by the social worker.

Social Work is a profession concerned with helping individuals, families, groups and communities to enhance their individual and collective well-being. It aims to help people develop their skills and their ability to use their own resources and those of the community to resolve problems. Social work is concerned with individual and personal problems but also with broader social issues such as poverty, unemployment and domestic violence.
